What information is included?

All licensed HMOs are included as of December 31, 2008, except health plans withdrawing from the Florida market. Also included are most non-managed-care health plans (PPOs and Fee-For-Service) with at least 5,000 covered lives in Florida in 2008. In some cases, no information is available because the health plan is new or has a small enrollment (not measurable). In other cases, the plan did not provide information (no report).

What information is not included?

Members of self-insured plans are not included in the report. Some self-insured plans are administered by HMOs. Ask your employer if you are not sure about what type of insurance you have.
